Transition Care Program (TCP): The TCP team specifically support older people who are recovering from injury or illness and who require a slower paced rehabilitation program to facilitate their return to independent living.
TCP can be provided in a number of smaller hospitals and hostels across the Albury Wodonga Health catchment area as well as directly within people's homes if appropriate. All referrals for TCP must be made via your local Aged Care Assessment Service.
