IMPORTANT Updates - Decline Form Process
Influenza vaccination is a requirement of employment.
Staff who want to do not want or cannot have an influenza vaccination MUST fill out the below form.
This requires a Manager risk assessment to discuss Staff and Patient safety and risk, and may require redeployment for those working in High-Risk Areas
(as defined in the VicHealth policy linked attached)
High risk work areas include:
- antenatal, perinatal and post-natal areas including labour wards and recovery rooms
- neonatal intensive care units and special care units
- paediatric intensive care units
- transplant and oncology wards
- haemodialysis units
- respiratory wards
- emergency departments
- intensive care units
- aged care wards and residential aged care settings.
If staff have not completed a form and have not had a vaccination in 2026 it will be reported to executive to advise on further action.
- If you have a medical exemption from your GP or on Medicare, please complete the Decline Form
- If you have had a 2026 influenza vaccination, elsewhere email us on InfectionControl@awh.org.au
- Clinics are scheduled and resourced to the best of our ability.
- Roaming clinics will occur but the need for a safe space for immunisation will be arranged in the ward areas.
- Please reach out if you or your team cannot access a clinic.
