Happy Care Coordinator Day

There are teams at AWH whose role is to Coordinate Care.
These teams include:
ASET – Aged Services Emergency Team
TCP - Transition Care Program
SITH – Sub acute in the Home
HCT - Health Coordination Team
STS - Short Term Supports
The staff in these teams have well developed skills and knowledge to arrange care, services, and support for clients. They can be either nursing or Allied Health background.
Coordinators Day Lunch was organised to celebrate the work that these staff do, and to informally meet each other and to start to learn the different aspects of work the teams do.
We have identified there are lots of commonalities, and many staff within the teams who don’t know each other. Looking forward to broadening our knowledge and networking with more staff and teams across AWH who coordinate care.
