Acute Geriatric Evaluation and Management (A-GEM): The A-GEM program operates at the Wodonga hospital and supports older patients who are experiencing functional decline as a result of acute illness or chronic and complex health conditions associated with ageing.
The A-GEM team provides comprehensive assessment and management of a person's physical, emotional, cognitive and functional abilities. The A-GEM team support people to return to a realistic level of independent living with the skills and supports necessary to self-manage their health and this often means patients are referred to other programs, such as the Transitional Care Program, for ongoing care.
