Social Work provides support for people with health related issues who require assistance with social supports and social issues.

This may include, but not be limited to, the areas of advocacy, accommodation, finance, adjustment/loss and relationship issues. The service provides phone and/or face-to-face contact providing assessment of needs with further management and/or referral to other services as appropriate. Ongoing or short term support is providing where required.

Social Work services are provided to all admitted services and community based programs within Albury Wodonga Health including Albury and Wodonga Hospitals, the Community Rehabilitation Centre (CRC), the Transition Care Program (TCP), Post Acute Care (PAC) and the Rural Allied Health Team. (RAHT)
