Rehabilitation

Inpatient Rehabilitation: The inpatient rehabilitation team, which consists of specialist, medical, nursing and allied health practitioners, support patients to recover from acute illness and injury and regain the functional skills required to return home. Patients are often referred to community based rehabilitation programs for ongoing care and support following discharge from inpatient rehabilitation.

Specialist Rehabilitation Assessment: Albury Wodonga Health provides a number of specialist outpatient rehabilitation clinics including: prosthetics and orthotics, spasticity assessment and management, complex neurological rehabilitation and general rehabilitation assessments. Following an assessment with a Rehabilitation Physician people may be referred for an inpatient admission, for community based rehabilitation, or continues to be reviewed by the Rehabilitation Physician.

Transition Care Program (TCP): The TCP team specifically support older people who are recovering from injury or illness and who require a slower paced rehabilitation program to facilitate their return to independent living. TCP can be provided in a number of smaller hospitals and hostels across the Albury Wodonga Health catchment area as well as directly within people's homes if appropriate. All referrals for TCP must be made via your local Aged Care Assessment Service.
