Rehabilitation

Rehabilitation

Rehabilitation Programs at Wodonga Campus aim to assist clients to achieve optimal independence and quality of life.

Rehabilitation is offered:

  • As an inpatient in the Medical/Rehabilitation Service
  • As a community - based service at the Centre or in the clients home


Inpatient Rehabilitation

Referral and Assessment

Referral can be made from the Acute wards or the Community.

Following assessment by the Program Coordinator or Rehabilitation Specialist, an admission date is planned.

Care Planning

The Rehabilitation programs offer integrated care including, Medical, Nursing and Allied Health.

Rehabilitation goals and progress are reviewed regularly during team meetings.  Admissions and discharge plans are initiated at the case conference.

Discharge plans are made in consulation with client and carer.  if required referrals are made to other appropriate therapy and / or community support services.

Allied Health Services

Physiotherapy

Physiotherapy assessment and treatment addresses such conditions as poor balance, immobility and coordination.  The Physiotherapist will advise regarding appropriate mobility aids, exercise programs, and lifting and transferring techniques for staff and carers.

Occupational Therapy

The Occupational Therapist works with the clients (and carers) to maximise safety and independence in daily tasks, e.g. self care, domestic chores and community access.  This may include recommendations for home modification and equipment, or teaching new methods of compensation to assist clients to return home.

Social Work

The Social Worker provides counselling and support for clients and carers.  Information referral or liaison with other services in the community is provided.

Speech Pathology

The Speech Pathologist assists client who are experiencing communication and/ or swallowing difficulties.

Dietician

A Dietician is available to assess and advise clients regarding their nutritional status and the role of food and diet in achieving optimal health.


Admission and Selection Criteria

Who will benefit from Rehabilitation

Individuals who have had a recent medical episode and require assistance to maximise their return to optimal functioning and independence, e.g. orthopaedic conditions, stroke or deconditioning following lengthy hospitalisation or illness.

Individuals who are at risk of losing independence and the ability to manage at them due to changes in physical/ cognitive/ social status.

Individuals who are likely to achieve greater independence and therefore lessen reliance on family and community services following a rehabilitation programs.

Selection Criteria

  • The client is medically stable and not requiring medical treatment that will significantly reduce particpation.
  • The client has the potential to benefit from multidisciplinary input and is likely to achieve significant gains in functional and psychosocial wellbeing.
  • The client has the potential to achieve an increased level of independence with input from Rehabilitation.
  • The patient is cognitively competent and capable of new learning.
  • The client is motivated and understands their reponsibilities in participating in a Rehabilitation Program.
  • The client may be in danger of premature admission to permanent care without input from a multidisciplinary team.
  • That the Rehabilitation program has the resources to adequately meet the client's particular needs.
  • The client is assessed as suitable for Rehabilitation by the Rehabilitation Specialist in consultation with the Program Coordinator.
  • Excessive pain limiting ability to participate in therapy (e.g. Chronic low back pain).


Client and Carer Rights and Responsibilities

Client and Carer Rights

  • To receive quality rehabilitation and care, on the basis of identified needs and mutually agreed goals.
  • To have full knowledge of treatment options, and to be directly involved in decision-making regarding your rehabilitation program and discharge plans.
  • To be treated with dignity, respect an understanding of your individual cultrual and language needs.
  • To have all information treated in a confidential manner, ensuring privacy for you and your family.
  • To expect fair and prompt action should problems or complaints occur.

Client and Carer Responsibilities

  • To activily participate and commit to your agreed rehabilitation program
  • To treat each staff member with respect and provide a safe environment in which we can care for you.
  • To fully inform staff of any information that may impact upon your rehabilitation.
  • To keep all appointment times or advise staff if you need to reschedule.
  • To take responsiblity for maintaing your health gains during and on completion of the program.
