Social Workers recognise that being hospitalised or having decreased independence can sometimes cause unexpected social, emotional, financial or practical problems in a patient's or family member's life.
As a result, Albury Wodonga Health - Wodonga Campus (AWHWC) Social Workers work collaboratively with clients, families and health care professionals to contribute to the psychosocial health and well-being of the patient (and their families).Social Workers are skilled professionals who can provide a comprehensive range of strategies, including counselling, advocacy, practical & emotional support, education and information, to people who may find it hard to adjust to changes in their health status or capabilities as a result of illness or injury.
Social Work Services
Social Workers provide services to all clinical and community programs of the AWHWC and are part of the multi-disciplinary teams, comprised of allied health professionals, nursing and medical staff. AWHWC Social Workers can provide assessment and assistance in relation to:
- Adjustment to loss of independence
(both in the community and / or after hospitalisation)
- Future planning
- Relationship difficulties
- Child and family issues
- Personal crisis or trauma
- Domestic violence
- Aged care or respite care
- Grief and loss issues
- Anxiety and depression
- Substance abuse issues
- Stress management
- Housing, financial or legal issues
- Cultural issues
Referrals
Patients, family members, community agencies and health care staff can make referrals by contacting the Social Work service or rehabilitation programs on the numbers listed below. Clients may also be identified through the admission screening process.
