Diabetes Educator
The community Diabetes Educator is available for one on one consultations for people with diabetes and impaired glucose tolerance. They play a key role in the management of an individuals diabetes by providing them with education on the fundamentals of diabetes, important components of effective management and education on how to measure and interpret blood glucose levels.
Effective diabetes management is a coordinated approach by the patient, their GP, diabetes educator, dietitian, podiatrist and optometrist or opthamologist. It is recommended that a person with diabetes visits their diabetes educator every 12 months to undertake and review their management.
The community Diabetes Educator is located in Vermont Street in the Community Rehabilitation Centre.
