Physiotherapist - Transition Care Program

40 hrs per fn

The Wodonga Campus of Albury Wodonga Health (formerly Wodonga Regional Health Service) seeks to employ an experienced Physiotherapist to provide services to clients of the transition care program (TCP).

The TCP team currently comprises one coordinator and an Allied Health Assistant providing slow stream rehabilitation to clients who have had a hospital admission and are not yet ready to go home & function safely and independently.  The program aims to prevent premature admission to residential care.

Wodonga has 4 TCP places, with an anticipated expansion to 16 places by late October 2010.  Our current bed-based TCP places are at Westmont Homestead, Baranduda.  Our home-based places are delivered in clients homes within the immediate Wodonga area.

The TCP brokers Occupational Therapy, Speech Pathology Social Work, Nursing and Dietetics services for clients. The team is supported by excellent administrative staff.
