The Transition Care Program (TCP) based at Wodonga Hospital is seeking to recruit a part-time Social Worker to the team.
TCP provides short term care and support to older people after an acute hospital admission. The aim o f the program is to optimise the client's function and independence through low intensity therapy, services and case management and assist people to return home after a hospital stay rather than entering residential care prematurely.
The Social Worker role involves working as part of the interdisciplinary team, with clients and their families to support client goal's and assist with discharge/future planning.
Wodonga TCP has 16 places in total; 8 are community based and 8 places are in bed based facilities located in Wodonga, Indigo and Towong Shire.
