Hospital Admission Risk Program - Chronic Disease Management

These positions provide an opportunity for appropriately skilled and experienced allied health and/ or nursing professionals to undertake an increasingly important role in the comprehensive management of people with chronic and complex conditions.

The role of a HARP Care Coordinator role is an exciting and challenging one. This is a growing area of practice, with a focus on client empowerment and self management.

The Care Coordinator undertakes comprehensive assessment and care coordination of individuals with chronic health conditions to enhance their health outcomes. Experience in chronic disease management is preferable, but not essential as training relevant to the role will be provided.

To succeed in this role, you will be enthusiastic and self motivated. You will gain satisfaction from supporting individuals with chronic disease to better manage their health in order to reduce avoidable hospital admissions, whether they have chronic heart disease, chronic respiratory disease, diabetes or all three.

The positions will provide other interventions and health promotion based on the professional qualifications and expertise of the successful applicants.

Responsibilities include assessment and care planning, facilitating and coordinating evidence based interventions relevant to the client's condition(s), liaison with the team and other services and input into the development of relevant Health Service programs, policies and research projects.

The successful applicants will have excellent interpersonal skills and be competent at working both autonomously and as part of a team. They will be able to develop and maintain strong and effective relationships between acute, subacute and primary care providers, particularly GP's and other community services. A sound understanding of chronic illness and conditions is essential.

Salary and conditions of employment will be according to qualifications and experience. It is essential that applicants, where relevant, hold current registration with their professional Registration board. A current drivers' licence is also a requirement.
