Intake, Assessment & Referral Worker
(Single Point of Entry)
Our Wodonga site operates a very busy “Single Point of Entry” for all referrals to ambulatory services – Community Rehabilitation, Continence services, HACC Allied Health, Community Health, HARP-CDM, Post Acute Care and the Transition Care Program.
The role of the SPE workers is to receive, assess and appropriately direct all referrals received to ensure the best outcomes for consumers. The SPE team includes experienced allied health clinicians and administrative support staff. They work closely with all clinical staff to constantly improve service access for the community.
This position requires sound clinical knowledge, an understanding of the relevant Department of Health funded programs and criteria, & excellent communication and interpersonal skills. Comprehensive training will be provided.
