An exciting and challenging opportunity exists for a part-time Care Coordinator within the HARP-CDM team at Wodonga Regional Health Service.
The position involves the assessment and coordination of care of individuals with chronic health conditions. Experience in chronic disease management is preferable, but not essential as training relevant to the role will be provided.
We are seeking an enthusiastic and motivated professional to join the HARP team. The aim of the HARP program is to assist individuals with chronic disease to better manage their health in order to reduce avoidable hospital admissions. Our program has a focus on people with chronic heart disease, chronic respiratory disease and diabetes.
Responsibilities include assessment and care planning, facilitating and coordinating evidence based interventions relevant to the client’s condition(s), liaison with the team and other services and input into the development of relevant Health Service programs, policies and research projects.
The successful applicant will have excellent interpersonal skills, be competent at working both autonomously and as part of a team and able to develop and maintain strong and effective relationships between acute, subacute and primary care providers. A sound understanding of chronic illness and conditions is essential.
Salary and conditions of employment will be according to qualifications and experience. It is essential that applicants, where relevant, hold current registration with their professional Registration board.
Possession of a current and valid drivers’ licence is a condition of appointment.
