Program Coordinator

We are offering an exciting and challenging opportunity for an appropriately skilled and experienced health professional to take up the role of Community Rehabilitation Program Coordinator.  The Program Coordinator is primarily responsible for the leadership and management of the multidisciplinary rehabilitation team in delivering a variety of clinical services.

The Community Rehabilitation service is available to people of all ages who may be referred by hospitals or from the community.  Clients may have a physical disability, be frail, chronically ill or recovering from a traumatic injury or illness.  Rehabilitation is aimed at regaining or maintaining optimal function and independence.

The Community Rehabilitation service is a “subacute ambulatory care service” (SACS) funded by the Victorian Department of Health as one of the suite of Health Independence Programs.  For detailed information regarding SACS please see

The successful applicant will be highly motivated, possess an excellent clinical knowledge base and have demonstrated ability to ensure best practice in service delivery.  They will have human resource management knowledge and skills, and an ability to develop and maintain strong and effective relationships between acute, subacute and other service providers.  Sound understanding of relevant Dep't of Health policy and guidelines will be advantageous.

Salary and conditions of employment will be according to qualifications and experience.  It is essential that applicants, where relevant, hold current registration with their professional Registration board.  A current drivers' licence is also a requirement.
