Hospital Admission Risk Program
Full time (Part time will be considered)
An exceptional career opportunity exists for a highly motivated Allied Health Professional or Division 1 nurse to join our dynamic HARP team.
Primary Responsibilities include providing care co-ordination to clients in the Upper Hume and Albury-Wodonga Region who have Chronic Health Conditions (COPD, CCF, Diabetes) with the aim of reducing unplanned hospital admissions and improving clients self management skills.
Ideally you will have excellent clinical knowledge and assessment skills, an understanding of the needs of people with chronic health conditions and complex needs, excellent communication, interpersonal and negotiation skills. You will have an understanding of the care across the continuum, flexibility and responsiveness to change and the motivation to make a difference.
Health coaching/motivational interviewing experience, and an interest in Advanced Care Planning/Directives would be considered an advantage, but not essential.
